What Happened

A 17-month-old child became entrapped in a head and neck opening on the playset and died. The CPSC identified the entrapment hazard: the design allows a toddler's torso to pass through certain openings in the unit while the head cannot, creating a dangerous trap. Young children — particularly toddlers in the 12-to-24-month range — are most vulnerable because they can push forward into an opening that cannot accommodate their head.
